Provide special support services to individuals and families and assist them in their progression to self-sufficiency by stabilizing and improving their housing, employment, family, and mental health. This practice is designed to address the consumers need through intense case management and the promotion of wellness and recovery. Clients will definitively obtain permanent independent housing upon a successful transition in Belmont Homes.
